Guide / Fractional IT
How does fractional IT work?
Fractional IT means you get a slice of a complete IT department, senior leadership and a support team, for a flat monthly fee, instead of hiring full-time staff you may not need all of. You get the expertise of a whole department at the fraction you actually use, and it scales as you grow. Here is how the model works, and what it looks like for a small, medium, and large organization.
How the model works
You buy the IT function you need, not the headcount you do not.
A full in-house IT department is expensive: a help desk, a systems engineer, a security specialist, and an IT leader can easily run past several hundred thousand dollars a year in salaries alone, before tools and training. Most small and mid-size organizations do not have enough work to keep all of those roles busy full-time, but they still need every one of those skills.
Fractional IT solves that. Instead of hiring, you engage a provider who assigns you a portion of an established team: a help desk for day-to-day issues, engineers for projects, a security lead, and a virtual CIO for strategy. You pay a flat monthly fee scaled to your size, and you dial the mix up or down as your needs change. You get senior expertise and 24/7 coverage that a single hire could never provide, for less than the cost of that hire. See our fractional IT services and fractional cybersecurity team.
Fractional IT by company size
The same model fits very differently depending on where you are. Here is how it works for each.
Small · roughly 1–25 staff
A whole IT function you do not have yet
Small offices and solo or small practices usually have no real IT, just whoever is least afraid of the computer. Fractional IT gives you the entire function at once: a responsive help desk, security and backups, HIPAA basics, and one accountable partner. You get enterprise-grade tools and coverage without a single hire, at a predictable monthly cost. You also get a technical account manager as your single point of contact, covered below.
Medium · roughly 25–150 staff
Leadership and depth on top of a stretched team
Mid-size and multi-location organizations often have one or two internal generalists who are buried. Fractional IT adds what they cannot cover: a virtual CIO for strategy and budgeting, a security lead, and project capacity for migrations, new sites, and audits. We can support your internal staff in a co-managed model, or run IT end to end. At this size you also get a dedicated engineer and a named point of contact, covered in detail below.
Large · roughly 150+ staff
Specialist seats and surge capacity
Larger and multi-location organizations have an IT team but real gaps in senior, specialized roles. Fractional IT fills specific seats, a virtual CISO, a compliance lead, a cloud architect, and adds surge capacity for big projects and audit seasons, without the cost and lead time of full-time executive hires. And if you already have internal IT, we can bring your team under our umbrella, covered below.
A dedicated point of contact at every size
However big you are, you get a real person who owns your account, not a faceless ticket queue.
For small organizations
A technical account manager who owns your needs
Even the smallest practice gets a technical account manager as a single primary point of contact. They master your environment and your goals, work alongside you as a tailored, high-touch partner, and keep your technology in step with where the practice is heading. It is the personal relationship a big-box IT provider cannot give a small office.
For medium organizations
A dedicated engineer and named point of contact
Past a certain size you do not just get a help desk, you get a dedicated engineer assigned to your account. They learn your environment, your software, and your people, and act as your primary technical contact alongside a named account lead, so you are never re-explaining your setup to a stranger in a queue.
This is the pod model the strongest providers use: a small, consistent team owns your account instead of a faceless ticket line. The payoff is faster response, fewer repeated explanations, and someone who already knows your EHR and your quirks the moment something breaks.
For large organizations with internal IT
We absorb your existing IT team
If you already employ IT staff, you do not have to choose between keeping them and gaining our depth. In a co-managed model we bring your existing team under our wing: our tools, processes, documentation, 24/7 monitoring, and senior escalation behind the people who already know your business, with a dedicated team that owns your account.
Your staff keep doing what they do best while we cover after-hours, surge projects, security, and specialist work they do not have bandwidth for. It is the established co-managed and rebadging model larger organizations use to scale IT without losing institutional knowledge or burning out their people. See our cybersecurity and managed IT.
Fractional IT vs managed IT vs full-time
They overlap, but they are not the same thing.
Fractional IT
A part of a team, including leadership
You get a portion of a full IT department, day-to-day support plus senior strategy (vCIO, vCISO), scaled to your size. Best when you need expertise and direction, not just fixes.
Managed IT (MSP)
Your operations, run for you
A managed service provider runs your IT operations for a flat fee: monitoring, help desk, security, and backups. Fractional IT is often delivered on top of this, adding the leadership layer.
Full-time hire
One person, one salary
An employee gives you presence and control, but one person cannot cover every specialty or 24/7. Expensive to scale, and a single point of failure when they are out or leave.
What you actually get
The roles a fractional engagement puts on your side.
🧠
Virtual CIO (vCIO)
Technology strategy, budgeting, and a roadmap tied to your growth and compliance calendar, reviewed with you each quarter.
🛡️
Virtual CISO (vCISO)
Security leadership: risk decisions, policy, and the cybersecurity program that satisfies HIPAA and your cyber-insurer.
📱
Managed help desk
24/7 monitoring and a responsive team for everyday support, with patching scheduled around your hours.
🔨
Project engineering
Migrations, new locations, EHR rollouts, and infrastructure work delivered without pulling your people off their jobs.

Fractional IT: questions, answered
What is fractional IT, in one sentence?
It is access to a complete IT team and IT leadership for a fraction of the time and cost of hiring them full-time, delivered on a flat monthly model and scaled to your size.
How is fractional IT different from a managed service provider?
A managed service provider runs your IT operations for you. Fractional IT adds the leadership and specialist layer on top, a virtual CIO and CISO and senior engineers, so you get strategy and direction, not only day-to-day support. We deliver both together.
What is a virtual CIO or virtual CISO?
A vCIO is a part-time IT executive who owns your technology strategy and budget; a vCISO does the same for security. You get the seniority of an executive hire for the hours you actually need, instead of a full salary.
What size company is fractional IT for?
All of them. A small practice gets an entire IT function it never had; a mid-size group gets leadership and depth on top of a stretched internal team; a large organization gets specialist seats and surge capacity. The mix is scaled to you.
What does fractional IT cost?
It is a flat monthly fee based on your size and the mix of roles you need, far less than the equivalent full-time salaries. See our cost guide, or book a free consultation for a scoped quote.
